New Patients & Referrals

Where can we have a referral sent?
Please fax a referral to (912)721-7933 or email We will need the patient’s insurance card and medical history along with the MD referral. We will then contact the patient directly to schedule an appointment.

If your physician has ordered an evaluation, or if you’d like to discuss how to receive a physician’s referral, please call us at (912)299-2394 or email Prior to scheduling your appointment, we will need a copy of your insurance card.

Which insurances do you currently accept?

Coastal Plain Therapy & Wellness is currently an in-network provider for the following insurance companies:

  • Medicare

  • Aetna

  • Ambetter

  • United HealthCare

  • Tricare

  • Anthem, Blue Cross Blue Shield


For patients with insurance companies other than those listed above: Out-of-network services are still covered by many insurance policies with prior authorization if the services are deemed medically necessary. We do our best to confirm benefits and seek authorization for coverage of services; however, it is YOUR responsibility to understand your plan and its coverage.


We also offers private pay services for qualifying patients. Call for more information!
